It appears as though you may not have a recorder set properly in Nero. You can check this first, by opening Nero Burning Rom (not Nero Express) and when you choose DVD on the top left of the window that opens, if it says "image" that is because you need to select a recorder. If this is the case, then choose recorder, close Nero, and start over by right clicking on the .iso again. It should never be asking you where to save the file, you are burning it to DVD.
Burning an .iso with Nero is very easy, provided your settings in Nero are correct first. Also make sure you are choosing Nero Burning Rom on the right click, and NOT Nero Express, then you choose burn image, and Nero opens, default should be 16x burn speed, and click on burn.

start > programs > Nero > Burning Rom > right click on Nero Burning Rom, send to, desktop, create shortcut. Then you do not have to get to it through Nero Smart Start and you have a direct shortcut right to the part of Nero you need to use.
2nd EDIT: Just to be sure, when you mouseover the file you are calling an .iso, does it have an image icon, and does it say "image file" when you mouseover it? Sometimes the .iso files are wrapped in winrar or other formats, and while they may be labeled as an .iso, it may be that you have to extract the .iso first prior to burning....just checkin' to be sure.
